Driver Qualification Files: What's Required and How to Track Them

Every carrier must maintain a Driver Qualification File (DQF) for each driver under FMCSA regulations. Incomplete or expired DQFs are among the top audit violations. This guide covers exactly what belongs in each file, common pitfalls, renewal schedules, and how to track compliance across your entire roster.

What Is a Driver Qualification File?

A Driver Qualification File (DQF) is an FMCSA requirement under 49 CFR Part 391. Every carrier that employs drivers operating commercial motor vehicles must maintain one DQF for each driver. The file demonstrates that the driver meets federal qualification standards and that the carrier has verified their employment history, driving record, and physical fitness.

During a DOT compliance review, auditors will pull a sample of your driver files and check each required element. Missing or expired documents can result in violations, fines, and in severe cases, an unsatisfactory safety rating. Keeping DQFs complete and current is non-negotiable.

Required Documents in a DQF

FMCSA specifies exactly what must be in every driver qualification file. Missing any of these items is a violation:

  • Driver application - Must include 10 years of employment history and 3 years of driving history. The application must be signed and dated by the driver.
  • MVR (Motor Vehicle Record) - Pulled annually from each state where the driver held a license in the prior 3 years. Documents traffic violations and license status.
  • Medical Examiner's Certificate (med card) - Valid for up to 2 years. Some conditions require annual renewal. The driver cannot operate a CMV with an expired med card.
  • Road test certificate or CDL equivalent - Proof that the driver passed a road test. A valid CDL serves as the equivalent for most drivers.
  • Previous employer safety performance history inquiries - Must be completed within 30 days of hire. You must contact each employer from the prior 3 years and document their response.
  • Drug & Alcohol Clearinghouse queries - Pre-employment query before the driver operates a CMV, plus annual queries for every driver. Results must be retained.
  • Annual review of driving record - The carrier must review each driver's MVR annually and document that the driver meets qualification standards. A signed form is required.
  • Background check - Criminal history and other background verification as required by your hiring policy and applicable state law.

Common DQF Audit Violations

FMCSA auditors see the same DQF problems repeatedly. Avoid these violations:

  • Expired med cards - Drivers with expired medical certificates cannot legally operate. Auditors check every med card date. Renew 30–60 days before expiration.
  • Missing MVRs - MVRs must be pulled annually. If you missed a year or never pulled from a state where the driver held a license, that is a violation.
  • No previous employer inquiries - Carriers must contact prior employers within 30 days of hire and document the inquiry. "Could not reach" is not sufficient without documented attempts.
  • Incomplete applications - Gaps in the 10-year employment history, missing signatures, or undated forms all count as violations.
  • Missing Clearinghouse queries - Pre-employment and annual Drug & Alcohol Clearinghouse queries are mandatory. No exceptions.

Renewal and Retention Schedules

DQF documents have different lifecycles. Knowing what expires when helps you stay ahead:

  • Medical Examiner's Certificate - Valid for up to 2 years. Drivers with certain conditions (e.g., diabetes, sleep apnea) may require annual certification. Set reminders 60 days before expiration.
  • MVR - Must be pulled at least annually. Many carriers pull MVRs on the driver's hire anniversary to simplify tracking.
  • Annual driving record review - Must be completed within 12 months of the previous review. The carrier signs a form attesting the driver meets qualification standards.
  • Drug & Alcohol Clearinghouse - Pre-employment before first dispatch; annual query for every driver. Schedule annual queries on a consistent date (e.g., hire anniversary).
  • Retention - Keep DQF records for 3 years after the driver leaves your employ. Some records (e.g., drug test results) have longer retention requirements under 49 CFR Part 382.
